Gotta be simple but I don't get it. - Ear Spiral

I have the feeling this is one of those things that's right in front of my nose - too closely to be seen. I am having the worst time trying to figure out how the outside spiral is formed around the inside beads. I can't get past the step of forming the curve that hangs in the earlobe. Help?

Are you on step 12?
The next step is to insert the beaded part into the pen barrel you were using as a mandrel. You need a hollow piece. I would suggest you to use the cap of that pen.
So you actually form the spiral AROUND the beaded part using the cap as a mandrel. The beads are inside of the cap while you are forming the spiral on the outside of the cap.
Of course the cap has to be wide enough to allow you to slip in and out the beads, but not too wide as to make it look weird.
